Critical Values — When to Act Immediately
Critical values are the thresholds where a lab result demands an immediate nursing response — notify the provider, hold a medication, or intervene at the bedside. NCLEX priority questions are built around these numbers. If you know the critical ranges cold, priority questions become pattern recognition.
Cardiac dysrhythmias — report immediately. K+ < 3.0: replace before diuretics. K+ > 6.0: cardiac emergency — calcium gluconate first, then insulin + glucose to shift K+ intracellularly.
Seizure and neurological risk. Hyponatremia < 120: high seizure risk, restrict free water. Hypernatremia > 155: cerebral dehydration, correct slowly to avoid cerebral edema.
< 50: hypoglycemic emergency — give 15g fast-acting carbs (conscious) or D50 IV (unconscious). > 400: DKA or HHS — fluids, insulin drip, electrolyte monitoring.
Likely transfusion threshold. Notify provider, assess for symptoms (fatigue, tachycardia, dyspnea). Prepare for packed RBC transfusion — type and cross-match, two large-bore IVs.
Bleeding risk — hold warfarin dose, assess for active bleeding (bruising, hematuria, black tarry stool), notify provider. Antidote: Vitamin K.

Knowing the numbers is only half the battle. The other half is recognizing which question type you are in and what the NCLEX is actually testing. Here are the four most common formats.
Priority Questions: “Which finding requires immediate action?”
You are given four patients — or four lab findings for one patient — and asked which one to address first. This is pure critical-value knowledge. A K+ of 6.4 beats a K+ of 5.8; an INR of 4.9 beats a glucose of 250. Know the hierarchy: immediate cardiac or airway threats first, then acute injury risks, then medication-safety concerns.
Medication Safety: Lab + Drug Interaction Questions
These are the most heavily tested lab questions on NCLEX-PN and RN. Classic pairings: low potassium + digoxin → toxicity risk (hold digoxin when K+ falls below 3.5); lithium levels therapeutic 0.6–1.2 mEq/L, toxic above 1.5; supratherapeutic INR + warfarin → hold dose and assess bleeding. NGN Scenarios: Lab Trend Interpretation
Next Generation NCLEX gives you a lab panel across two or three time points and asks you to interpret the trend. Rising creatinine from 1.1 → 1.8 → 2.6 over 48 hours is AKI in progress. Unlicensed assistive personnel (UAP) can collect specimens and report numbers to the RN — but they cannot interpret lab values, and they cannot act on them. How to Actually Study Lab Values for the NCLEX

Group your labs: kidneys (BUN, creatinine, urine specific gravity), liver (ALT, AST, albumin, bilirubin), blood (CBC — WBC, Hgb, Hct, platelets), heart (cardiac enzymes in context, electrolytes for dysrhythmia risk), lungs (ABGs). When you see a lab in a question, your first thought should be “which organ is this measuring?”
Learn the “bad direction” for each value
Every lab value has a dangerous direction: creatinine goes up in kidney failure; hemoglobin goes down in anemia and bleeding; platelets go down in thrombocytopenia; potassium goes too high in renal failure and too low with diuretics. Instead of memorizing both ends of every range, ask: “Which direction is the emergency?” That narrows what you need to know for most NCLEX scenarios.
Always pair labs with conditions, not in isolation
Renal failure means high BUN, high creatinine, high K+, low Hgb (erythropoietin deficiency), and low Hct — all at once. Liver failure means elevated ALT/AST, low albumin, elevated bilirubin, and prolonged PT/INR. DKA means high glucose, low bicarbonate, high K+ (despite total body deficit), and metabolic acidosis on ABG. Studying labs in clinical context makes them stick — and makes NCLEX scenarios recognizable.

When you see a lab trend in an NGN scenario, ask yourself two questions before selecting your answer: “What organ is this lab measuring?” and “What is the consequence of this value going further in this direction?” A creatinine rising from 1.2 to 2.8 tells you kidneys are failing — the consequence of that trajectory is metabolic acidosis, hyperkalemia, fluid overload, and uremia.
