Why Cardiovascular Is a Make-or-Break NCLEX Category
Cardiovascular content accounts for a disproportionately large share of NCLEX questions. When you look at the test plan categories of physiological adaptation and basic care and comfort — which together represent roughly 17–23% of the exam — cardiac conditions, hemodynamic stability, and cardiovascular medications run through nearly every scenario. That means a student who freezes on cardiac rhythms or blanks on which beta-blocker parameter to hold is leaving significant ground on the table before the exam is half over.

Heart Failure: Systolic vs. Diastolic, S3 vs. S4
Systolic heart failure (reduced ejection fraction) means the heart cannot pump effectively — output is too low. Key signs: fatigue, dyspnea on exertion, S3 gallop (the sound of blood sloshing into a dilated, poorly contracting ventricle), and bilateral crackles from pulmonary congestion. Diastolic heart failure (preserved ejection fraction) means the heart cannot relax to fill adequately — pressure backs up even though the squeeze is intact. Signs are similar but an S4 gallop (atrial kick against a stiff ventricle) is the distinguishing sound.
Management priorities on NCLEX: Elevate the head of bed 45° to reduce preload. Administer oxygen to maintain SpO2 ≥ 94%. Daily weights are the #1 monitoring intervention — report a gain of more than 2 lb in 24 hours or 5 lb in a week. Restrict sodium and fluids as ordered. Assess for peripheral edema, JVD, and lung sounds bilaterally.
Myocardial Infarction: STEMI vs. NSTEMI and MONA-B
STEMI (ST-elevation MI) involves complete coronary artery occlusion with full-thickness myocardial damage — this is the cardiac emergency requiring emergent PCI (cath lab) within 90 minutes of first medical contact. NSTEMI involves partial occlusion with partial-thickness damage — troponin is elevated but no ST elevation on 12-lead ECG. Both require immediate intervention, but the urgency and pathway differ.
MONA-B mnemonic for priority interventions:
- M — Morphine (for unrelieved pain, IV)
- O — Oxygen (if SpO2 < 94%)
- N — Nitroglycerin (sublingual, max 3 doses q5min; hold if systolic < 90)
- A — Aspirin (324 mg chewed — non-enteric-coated for rapid absorption)
- B — Beta-blocker (initiated within 24 hours to reduce myocardial oxygen demand)
NCLEX classic: the patient who reports chest pain radiating to the left jaw, diaphoresis, and nausea — this is until proven otherwise an MI. Priority nursing action: obtain a 12-lead ECG immediately and notify the provider. Do not leave the patient.
Hypertension: Crisis vs. Urgency
Both involve systolic BP > 180 or diastolic > 120 — the difference is end-organ damage. Hypertensive crisis (emergency) involves acute end-organ damage: encephalopathy, acute MI, aortic dissection, pulmonary edema, or eclampsia. This requires IV antihypertensives (labetalol, nicardipine, nitroprusside) and ICU-level monitoring — BP must be reduced in minutes to hours. Hypertensive urgency is severely elevated BP without end-organ damage — treated with oral agents over 24–48 hours (no rush; overly rapid reduction can cause ischemic stroke or MI).
NCLEX nursing tip: When a question gives you a BP of 210/118 — your first assessment step is to evaluate for end-organ damage symptoms: headache, visual changes, chest pain, confusion, hematuria. Those symptoms determine whether you are dealing with an emergency or urgency scenario.
Cardiac Dysrhythmias: A-fib, V-tach, V-fib
Atrial fibrillation (A-fib): Irregularly irregular rhythm, no identifiable P waves. Key nursing concerns: rate control (beta-blockers, diltiazem), anticoagulation (warfarin or DOAC) to prevent clot formation from atrial stasis, and stroke risk. INR target for A-fib on warfarin is 2–3. If a new-onset A-fib patient is going to cardioversion, they must be anticoagulated for 3 weeks prior or have a TEE ruling out atrial thrombus.
Ventricular tachycardia (V-tach): Wide complex tachycardia, rate 100–250. If the patient has a pulse and is hemodynamically stable — call the provider, prepare for medication (amiodarone IV). If pulseless V-tach — this is a cardiac arrest: start CPR, prepare for defibrillation immediately. The pulse check is the pivot point.
Ventricular fibrillation (V-fib): Chaotic, no recognizable complexes, no cardiac output. This is always a cardiac arrest — start CPR immediately, call for a defibrillator, and deliver an unsynchronized shock as soon as available. V-fib does not wait. Do not check a pulse first if you see V-fib on the monitor and the patient is unresponsive — begin CPR.
Peripheral Vascular Disease: Arterial vs. Venous Insufficiency

High-Yield Cardiac Medications: What NCLEX Tests
Cardiac pharmacology questions follow a predictable pattern: the NCLEX gives you a patient, a drug, and a current assessment finding — and asks what you do next. For each medication below, memorize the hold parameter, the key side effect, and the antidote or reversal agent where applicable.
Hold if heart rate < 60 bpm or systolic BP < 90 mmHg. Beta-blockers reduce heart rate, myocardial oxygen demand, and mortality in heart failure and post-MI. NCLEX classic: a patient's resting HR is 54 — hold the metoprolol and notify the provider. Also watch for bronchospasm in patients with asthma or COPD (beta-1 selective agents are safer but not risk-free). Do not abruptly discontinue — rebound tachycardia and angina can occur.
Three key NCLEX side effects: (1) Dry, persistent, non-productive cough — switch to an ARB (losartan) if intolerable. (2) Hyperkalemia — ACE inhibitors block aldosterone, which normally excretes potassium; monitor K+ closely, especially with potassium-sparing diuretics. (3) Angioedema — rare but life-threatening; discontinue immediately. Hold for rising creatinine (signs of renal failure) and notify provider. Contraindicated in pregnancy (teratogenic — Category D/X).
Therapeutic range: 0.5–2.0 ng/mL. Classic toxicity signs on NCLEX: nausea and vomiting, bradycardia, and yellow-green halos in vision (xanthopsia). Also watch for dysrhythmias. Hypokalemia potentiates digoxin toxicity — even a therapeutic digoxin level becomes toxic when K+ falls below 3.5. Always check potassium before giving digoxin. Hold if apical pulse < 60 bpm. Antidote: Digibind (digoxin immune Fab).
Furosemide is a loop diuretic — it causes potassium loss (hypokalemia), so monitor K+ closely and teach the patient to eat potassium-rich foods (bananas, orange juice, potatoes). Also monitor for sodium, magnesium, and calcium losses. Spironolactone is a potassium-sparing diuretic — it retains potassium, so monitor for hyperkalemia (K+ > 5.0 mEq/L). Do NOT combine spironolactone with potassium supplements without close monitoring. NCLEX tip: furosemide + digoxin = double hypokalemia risk (furosemide drops K+, which raises digoxin toxicity risk).
Heparin (IV): monitor aPTT (goal 60–80 sec, or 1.5–2.5× normal). Antidote: protamine sulfate. Warfarin (Coumadin): monitor INR — therapeutic target for A-fib is 2–3; for mechanical heart valves, 2.5–3.5. Antidote: Vitamin K (slow reversal) or fresh frozen plasma (rapid reversal). NCLEX classic: patient on warfarin for A-fib has an INR of 4.2 — hold warfarin, assess for bleeding (gums, urine, stool, bruising), and notify provider. Teach warfarin patients: consistent vitamin K intake (do not suddenly change leafy greens), avoid aspirin/NSAIDs, report unusual bruising.
4 NCLEX-Style Cardiovascular Practice Questions
Work through each question before reading the rationale. Notice the pattern: the question gives you a finding, and your job is to match that finding to the correct clinical action.
Question 1 — Standard Multiple Choice
A nurse is preparing to administer metoprolol 50 mg PO to a client with heart failure. Assessment findings include: HR 56 bpm, BP 112/70 mmHg, SpO2 97%, RR 16. Which action should the nurse take?
- A. Administer the medication as ordered
- B. Hold the medication and notify the provider
- C. Administer the medication and reassess in 30 minutes
- D. Hold the medication and obtain a 12-lead ECG

Beta-blockers are held when HR is below 60 bpm or systolic BP is below 90 mmHg. This patient's HR of 56 is below the threshold. A 12-lead ECG (option D) is not indicated here — this is a medication-safety scenario, not a dysrhythmia emergency. The correct action is to withhold the dose and notify the provider before administering.
Question 2 — Priority (SATA)
A client with atrial fibrillation is on warfarin. The nurse reviews the morning labs: INR 4.8, HR 88 bpm, BP 134/82 mmHg. The client reports no active bleeding. Select all nursing actions that apply.

SpO2 of 89% and RR of 28 with orthopnea indicate acute respiratory compromise — the three immediate priorities are airway/breathing interventions: upright positioning reduces preload and improves diaphragmatic excursion (A), supplemental oxygen corrects hypoxia (B), and furosemide reduces the fluid overload driving the pulmonary edema (C). Daily weight (D) is important for monitoring but is not an immediate intervention during acute decompensation. Encouraging fluids (E) is contraindicated — this patient is volume-overloaded; additional fluids would worsen the condition.
Question 4 — Standard Multiple Choice (Digoxin Toxicity)
A client taking digoxin for heart failure reports nausea, loss of appetite, and seeing “yellow halos around lights.” The nurse reviews the chart: digoxin level 2.4 ng/mL, K+ 3.2 mEq/L, HR 52 bpm. Which action should the nurse take first?
- A. Administer Digibind (digoxin immune Fab) as ordered
- B. Hold the digoxin dose and notify the provider

This patient has three classic signs of digoxin toxicity: nausea, visual changes (yellow-green halos), and bradycardia — plus a supratherapeutic level (2.4 ng/mL, above the 0.5–2.0 range) and hypokalemia (K+ 3.2, which potentiates toxicity). The immediate nursing action is to hold the medication and notify the provider. Digibind (A) may be ordered, but administering it without a provider order is outside nursing scope. Potassium replacement (D) is appropriate but is a secondary intervention — the provider must order both. Never administer any medication without an order in response to toxicity.
Clinical Judgment Tip: Cardiovascular Priority Questions
Cardiovascular priority questions — “which patient do you see first?” or “which finding requires immediate intervention?” — are answered using two frameworks in order: ABCs first, then Maslow's hierarchy.
The Cardiovascular Priority Rule
When you have four cardiac patients, prioritize the one with the most compromised perfusion. Signs of compromised perfusion: altered mental status (the brain is the first organ to show decreased cardiac output), skin that is cool, clammy, and diaphoretic, SpO2 dropping, hypotension (systolic < 90), and new-onset chest pain or crushing pressure. Any of these signs means the patient's cardiac output is failing to meet tissue oxygen demand — that patient is your first priority.
Step 1: Apply ABCs — Is the Airway, Breathing, or Circulation at Risk?
A patient in V-fib (no circulation) beats a patient with A-fib (rhythm but low cardiac output) every time. A patient with SpO2 of 85% and respiratory distress beats a patient with 3+ edema and a normal sat. Start with the most acute threat to life — cardiac arrest, hemodynamically unstable rhythms, and acute respiratory failure always come first.
Step 2: Among Stable Patients — Who Has the Newest, Most Acute Change?
NCLEX will give you four patients where none has an obvious emergency. In that case, prioritize the patient with the newest or most acute change from baseline. A patient who just developed chest pain outranks a patient who has had stable angina for three days. A patient newly reporting palpitations with a dropping BP outranks a patient with chronic edema.
Step 3: Safety First When Two Patients Seem Equal
If two patients have equivalent physiological stability, apply Maslow — safety needs (fall risk, medication safety, infection control) rank above comfort needs (pain, sleep) and psychosocial needs (anxiety, teaching). The patient about to receive a medication that could cause torsades de pointes outranks the patient who wants discharge teaching about a low-sodium diet.
